I did not find any free/open-source apps, but MS does have the HTML Help SDK, available from: http://msdn.microsoft.com/library/default.asp?url=/library/en-us/htmlhelp/html/vsconHH1Start.asp which apparently will take the .hhp file generated by the XSL stylesheets (along with the other files) and create a .chm file. So it appears to do what I need. But I would really love to find one that works on Linux...
