Should I file a bug report > on SourceForge? Same output with 1.50 and 1.51.1. Yes, this is a bug, so please report it on SourceForge. Only certain elements trigger processing of footnotes with the 'process.footnotes' template, and sometimes those elements can be nested, resulting in footnotes being processed twice. Your article/glossary is one example, and chapter/refentry is another.
