Hmm, Jens's suggestion is basically what the stylesheet currently does. If there is a fileref value, it just copies it to the <img> element's src attribute without any additional processing. I find it quite strange that a piece of the source filename got in there. Are you using a customization? If so, did you try the stock stylesheet without the customization? What XSLT processor are you using? Any other details?
