Because the XHTML output is in a namespace. The <html> element in your XHTML output should have: <html xmlns="http://www.w3.org/1999/xhtml"> Your customization doesn't identify a namespace for its output, so the XSLT processor puts the output for your elements into an unnamed namespace to distinguish it from the xhtml namespace. What you want is for your elements to be in the xhtml namespace too.
