Ah, ok, I don't think I want to get into that ongoing discussion.

Regarding your stacked callouts, that is a result of the calloutlist being 
formatted with a fo:list-block with a narrow width to the 
fo:list-item-label:

provisional-distance-between-starts="2.2em"
provisional-label-separation="0.2em"

There isn't room for two graphics side-by-side, so you get line breaks. 
You would have to customize the template with match="calloutlist" in 
fo/lists.xsl to make the distance wider.

You could file a bug report to have the template count the maximum number 
of arearefs in a given callout, and adjust the width to fit the max.
