This is a bug in FOP. Here is
what the FO link looks like:
<fo:block>my entry,
<fo:basic-link
internal-destination="id428223">
<fo:page-number-citation ref-id="id428223"/>
</fo:basic-link>
</fo:block>
The link text is an empty
<fo:page-number-citation> element whose content is generated by the
XSL-FO engine. But FOP doesn't make it a link. If you put any
actual text inside the fo:basic-link element, it will form the link on the
text you add, but the page number is still dead.

and the Apache FOP.

Try clicking on the page
numbers. They should link to the correct page. The text of
an indexterm is not a hot link because it can have many page
references.
