Are you using an .eps image or a bitmap? I've found that to use a bitmap on a cover I have to use xep's extensions to make the image the same size as the page.width. For this reason, I prefer to use epses. I don't know if this will work, but did you try setting the background-position-horizontal to a negative value, equal to the left page margin? Same with the background-position-vertical and the top margin.
