VIRTIO-125: block: badly aligned section levels

Some sections for the block spec are badly aligned:
- configuration layout is hidden under "Feature Bits" - should be separate sections
- requirements for device operation are seperate sections - should go under "Device Operation"

The TC accepts the following proposed changes to the specification:

Move legacy configuration layout out from under "Feature Bits".
Make device operation requirements subsubsections under "Device Operation".
